Unable to add module, "Error: 1 error occurred: * pq: column "provider_id" of relation "module_provider_deps" does not exist"

When I attempt to add modules to my organization, I am getting the following error,

Error: 1 error occurred: * pq: column "provider_id" of relation "module_provider_deps" does not exist

I am adding this module through Azure Devops Services. I have confirmed the service account is setup with the correct access.

I have verified I am following the correct structure for the modules I attempting to add.

If I were to remove a module I added only yesterday and re-submit it, I get the same error. There seems to be an issue with Azure Devops Services and Terraform Cloud.

Issue has been resolved by Terraform Cloud support.
They reported the bug and were able to resolve the issue within a few hours.
